Please bring your vision and medical insurance cards into your appointment, along with any required insurance referrals.

In addition for all appointments, bring current eyeglasses and contact lens boxes.

Cancellation Policy

Please cancel/reschedule your visit with at least 12 hours’ notice. A $50 no show fee will be assessed without 12 hours’ notice; same day reschedule is also considered a no show. No show fees must be paid before you are allowed to schedule another appointment. We try to provide a computerized reminder text for confirmations 1 month, 5 days and 2 days prior to your appointment; however, reminder calls are a courtesy and it is your responsibility to remember any appointments you have scheduled.
